National Diabetes Prevention Program (DPP)
This workshop has two components: a core set of classes that meet for one hour, weekly for 16 weeks, then a post-core that meets monthly for the remainder of the year. The goal is lifestyle change that results in a reduction in weight of seven percent with an increase in activity to 150 minutes per week. Topics include; nutrition, exercise, stress management, and lifestyle changes that will promote better health. Developed by the Centers for Disease Control and Prevention, the primary goal is to reduce the incidence of type 2 diabetes.
Living Well with Diabetes
Also known as Diabetes Self-Management Program (DSMP) This program is designed for people with type 2 diabetes and how to live well. Topics include: techniques to deal with the symptoms of diabetes, fatigue, pain, hyper/hypoglycemia, stress, depression, anger, fear and frustration; appropriate exercise for maintaining and improving strength and endurance; healthy eating; appropriate use of medication; and working with health care providers.
Living Well with Chronic Conditions
Also known as Chronic Disease Self-Management Program. Participants learn how making small changes in their lifestyle will lead to living better with their chronic disease.Topics include: techniques to deal with emotions such as pain, fatigue, isolation, and frustration; exercises to maintain and improve flexibility, strength, and endurance; communicating effectively with others, including health professionals; appropriate use of medications; and making better nutrition choices.
Living Well with Chronic Pain
This Chronic Pain Self-Management Program is designed for people who have a diagnosis of chronic pain and offers concepts on how to deal with their symptoms. Topics include: techniques to deal with frustration, fatigue, isolation, and poor sleep; appropriate exercise for maintaining and improving strength; appropriate use of medications; communicating effectively with family, friends, and health professionals; nutrition; pacing activity and rest; and how to evaluate new treatments.
Health Coaching for Hypertension
This is a class designed to assist individuals to manage and reduce their hypertension.
The sessions include information on hypertension, monitoring blood pressure, nutrition for managing and reducing hypertension, exercise, stress management, and other lifestyle factors. There are weekly action plans with group support and discussions.
A Matter of Balance
A Matter of Balance curriculum is designed to reduce the fear of falling and increase the activity levels of older adults who have concerns about falls. Activities include helping participants set realistic goals to increase their daily activity level, changes to their environment to reduce fall risk factors, and learning simple exercises to increase strength and balance.
Tai Ji Quan: Moving for Better Balance
Tai Ji Quan: Moving For Better Balance helps older adults improve their balance and reduce the likelihood of falling. Research has shown that people who complete the program are half as likely to fall and are less fearful of falling. Participants learn balance skills, good body alignment and coordinated Tai Chi movements.
Stay Active and Independent for Life (SAIL)
This evidence-based fall prevention program centers around a strength, balance and fitness program for adults 65 and older. Performing exercises that improve strength, balance, and fitness are the single most important activity that adults can do to stay active and reduce their chances of falling. The exercises can be done standing or sitting
Walk with Ease (WWE)
The Arthritis Foundation’s six week Walk With Ease program teaches participants how to safely make physical activity part of their everyday life. The program includes a guidebook and a walking schedule to get them safely moving toward better health. Trained exercise leaders begin each session with a pre-walk discussion covering a specified topic related to exercise and arthritis, followed by a 10 to 40 minute walk that includes a warm-up and a cool-down.
Aging Mastery
Aging Mastery aims to help millions of baby boomers and older adults take key steps to improve their well-being, add stability to their lives, and strengthen their ties to communities. Participants create their own playbook for aging well via actionable goals, sustainable behaviors, social engagement, and gratitude. Aging Mastery is a comprehensive and fun approach to living, that celebrates the gift of longevity. Central to the Aging Mastery philosophy is the belief that modest lifestyle changes can produce big results and that people can be empowered to cultivate health and longevity.
Powerful Tools for Caregivers
Powerful Tools for Caregivers is a six-week educational program that addresses the needs of caregivers. Program participants develop a wealth of self-care tools to: reduce personal stress; change negative self-talk; communicate their needs to family members and healthcare or service providers; communicate more effectively in challenging situations; recognize the messages in their emotions, deal with difficult feelings; and make tough caregiving decisions.
PEARLS
PEARLS (Program to Encourage Active, Rewarding Lives) is a treatment program designed to reduce symptoms of depression and improve quality of life among older adults. You will learn the skills to manage unpleasant feelings and improve your quality of life.
